Coordination
Care Coordination
is a care planning service available to adults in Anchorage who need assistance
locating and accessing community services for themselves or for their
loved one. Care Coordination services are designed to enhance a persons
independence, safety and comfort by finding and coordinating the help
he/she needs.
Our Care Coordinators are professionals who have extensive and current
knowledge of community resources. They know eligibility requirements and
payment options for long-term care alternatives and can assist you with
planning and decision making.
The Care
Coordination department is currently offering a "Loan Closet"
for the Agency's clients. Items are available free of charge to those
individuals who are ill or frail and need aids to assist them in their
daily lives. To learn more about the "Loan Closet" and to find
out what is currently available, call the Care Coordination Program
Assistant at 561-3313.
