Mini Grants
Money for services and items is available for people with
Alzheimers disease and related disorders (ADRD). The mini-grants
must provide to individuals diagnosed with ADRD essential items which
will directly improve their quality of life and increase their independent
functioning. These can include medical, dental, vision, hearing services,
supplies, therapeutic devices, adaptive equipment, and accessibility improvements
or supplies and services not available from any other funding source.
A person may apply for a mini-grant as a Beneficiary (person affected
by the disorder) or as an Applicant (a family member, care coordinator,
or guardian filling out the form on behalf of the Beneficiary). The mini-grant
program is funded by the State of Alaska’s ADRD Education and Support
Grant.
1. Submit a completed mini-grant application.
