It can be difficult and overwhelming determining what services are available and how to access them. Care Coordination services relieve some of that burden by guiding you through the maze of health care, financial and social services available in Anchorage, Bethel and Dillingham.

A variety of programs and services are available in the community to assist elders of all income levels and varying degrees of frailty. Using a care coordinator ensures that you or your loved one knows all the options available and are utilizing the right combination of services to receive the best care possible.

Not all services are available statewide, contact your local office to find out options are available in your area.

Care Coordination services are for those who require more than one service at a time or for those who need help accessing the Alaska Medicaid Waiver Program.

Our Care Coordinators

are professionals who have extensive and current knowledge of community resources. They know eligibility requirements and payment options for long-term care alternatives and can assist you with planning and decision making.

All Care Coordinators at Alzheimer’s Resource of Alaska are certified by the state and have specialized knowledge of dementia care resources.

Care coordinators work as the link between you or your loved one and the numerous services they receive.
