Tess

Care Coordination Supervisor
Born and raised in Juneau, Tess is familiar with the resources that are available and has strong community connections, which at times has been extremely beneficial to helping coordinate services and care.

Following graduation from Lewis and Clark College with a BA in Psychology Tess began working as a case manager/service coordinator at a local providing agency, working specifically with individuals who experienced complex medical conditions. Prior to that, a behavioral health specialist, providing direct care to individuals, who experienced mental health disorders, in group homes.

As a care coordinator, Tess is an advocate to individuals who are elderly and experience dementia-related symptoms, and to those who experience physical and/or intellectual disabilities. In her role, with support from the care coordination team, she writes the individual’s annual waiver for habilitative services, which helps ensure they have enough support so that they can live their own best life.

Tess is passionate about helping others.
