If you or a loved one are on Medicaid Waiver or is Medicaid Waiver eligible and are living with Alzheimer’s disease, related dementia, or another qualifying disability, navigating care options can be complex. Care Coordination Resource Alaska (CCRA) is here to guide you through the process so you can access the services you need with clarity and support.

What Is Care Coordination?

Care Coordination connects individuals and families to services that promote safety, comfort, and well-being. CCRA’s Care Coordinators work alongside you to develop a personalized care plan that aligns with your needs and living situation.

We help eligible Alaskans:

  • Access Home and Community-Based Services Waiver (HCBW) Services

  • Coordinate care including services available through the Medicaid Waivers and Community First Choice Program.

  • Monitor services and make adjustments over time

  • Advocate for your needs and support your goals

Please note: To receive Care Coordination services, individuals must be eligible for Alaska’s Medicaid Waiver program. If you're unsure about eligibility, our team can help you understand the process and next steps. If you do not qualify for Care Coordination, Care Navigation may be the right fit.

Who We Serve

Our Care Coordination services are available in Anchorage and Mat-Su to:

  • Alaskans of all ages who qualify for a Medicaid Waiver due to a physical, developmental, or intellectual disability

  • Adults living with Alzheimer’s disease or other dementias who meet Waiver criteria

Medicaid Waiver eligibility is based on meeting either Nursing Facility Level of Care (NFLOC) or Intermediate Care Facilities for Individuals.

We also assist individuals who are exploring Medicaid Waiver eligibility or transitioning from other support systems.

When you work with ARA’s Care Coordination team, you’ll receive:

  • One-on-one support to assess your needs

  • A custom care plan built with your input and preferences

  • Ongoing coordination with service providers, caregivers, and state programs

  • Advocacy and updates as your needs evolve

  • Culturally responsive care that reflects the unique challenges faced by Alaskans across the state

What is a Medicaid Waiver?

A Medicaid waiver in Alaska helps eligible individuals who need long-term care—such as those with Alzheimer’s, developmental, physical, or medically complex disabilities—receive services in their homes or communities instead of institutions, through a multi-step process involving eligibility determination, coordination with an ADRC or DDRC, and assistance from a Care Coordinator to complete applications and develop a support plan.
