Care Coordination is a partnering process that assists Alaskans in need of supports to live safely and independently as possible in the home of their choice. Our Care Coordinators are responsible for finding, obtaining, and coordinating services for our clients. Assessing each individual’s situation, our Care Coordinators make certain the services received meet the needs of our clients and families.

Each day, the Care Coordination staff demonstrates their competency in: aging issues, disability issues, community services, chronic disease management, benefits, legal and ethical concerns, and special needs.
