Weis - Care Coordinator

Erin joined our Care Coordination team in April, 2021. She comes to CCRA with 13 years of experience working in Health & Social Services, bringing with her a vast knowledge of Medicaid Home & Community Based Services and Personal Care Supports.

Erin started her career as a Direct Support Professional in 2008 and has served in various roles over the years, her favorite role has been a Care Coordinator for individuals on the ALI, APDD, CCMC and IDD Waivers. In Erin’s free time she enjoys being with her family, working in her garden and baking. Erin holds a special place in her heart for our elders and individuals who experience disabilities, she is very excited to reenter Care Coordination and feels blessed to continue on her journey of helping others.

Studie - Care Coordinator

Tazz has worked in the Care Coordination field for 7 years as direct support staff, as a Case Manager as well as a Case Manager Supervisor. She enjoys working with individuals with developmental and emotional disabilities as well as with elders. Tazz is very versed and knowledgeable in Personal Care Services and has a passion to help others live as independently and safe as possible in the community of their choice.

Born and raised in Palmer Alaska, when she is not working she enjoys watching movies, camping, fishing, swimming, playing basketball, enjoying local music in the park and painting rocks with her grandmother. The most common question she gets asked: “is Tazz your real name and is it short for anything?” Tazz is her real first name and it is not short for anything. Tazz looks forward to working with her new clients and building relationships with them in the years to come.

Randy
Hart - Care Coordinator

Randy served in the U.S. Army for eight years. While he served, he developed a strong passion to help, serve, and give back to his community.

Prior to joining CCRA 1 year ago, Randy was a Community Programs Coordinator that worked with severely emotionally disturbed children and adolescents at AK Child & Family. He has also worked as a Direct Service Provider at Mat-Su Services for children and adults and as a camp counselor and kayak guide. Because of his previous personal and employment experience he understands how frustrating and time-consuming it can be to navigate the processes needed to discover and be able to utilize available community resources. It is Randy’s goal to make those processes and resources as easy as possible to understand, navigate, and obtain for the clients he serves.

Randy has a Bachelor’s Degree in Sociology from the University of Alaska Anchorage, and he is always interested in hearing a good joke, pun, or attempting to make someone smile and brighten their day!

Massman - Care Coordinator

Jessica has been providing care coordination through CCRA for over 2 years. Prior to joining CCRA Jessica cultivated 6 years of experience serving community members in Alaska working at United Way, Covenant House, and STAR.

Through Care Coordination, Jessica helps connect individuals with the services they need to lead safe and independent lives, with focus on the ALI and IDD waivers as well as those individuals who may not qualify for a waiver yet, but who need in-home services. Jessica is certified in all waivers.

Jessica has a Bachelor of Human Services from UAA and is forever grateful that she chose the helping path.

Tanner - Care Coordinator

As a Care Coordinator, Olga work with participants who need support to live safely and as independently as possible in the home of their choice by assisting them with getting services to maintain quality of life. She ensure that the participants have a choice, safety, independence, and dignity in-home and community-based living with quality services to promote independence and incorporate each individual’s culture and value system.

Prior to joining CCRA in 2016 Olga was a Care Coordinator for Mat-Su Senior Services for 5 years. She also has experience as a nationally certified medical assistant, a PCA, and as a house manager at an assisted living facility for 4 years.

Donny
Donny
Cox - Care Coordinator

Donny Cox has been a Care Coordinator since January 2018. He feels he helps individuals and their families navigate a complex system to obtain assistance with their every day physical needs and advocate for their wellbeing.

He has:

  • Worked with the Medicaid state of Alaska programs since 2005
  • Overseen Personal Care Assistant Agencies, worked with the IDD population and group homes
  • Facilitated openings of Assisted Living homes
  • Worked with the State of Alaska Senior and Disabilities Services to obtain a view from the state’s side.

Donny went to school in Tempe Arizona and has always felt a bond with the IDD community. He feels he has a sense of calm and connection to assist and advocate for individuals.

Bartz - Care Coordinator

As a parent of a child who needs extra care, Amber has acquired the skills to advocate for and assist families on a very personal level. She strives to be a resource to those she works for and always listens to both the words that are said and the ones that aren’t.

Amber believes that family is an integral part of the team and attempts to give them a voice whenever she can. She has spent 13-years providing suggestions and assisting with accessing or changing services as well as linking people to grants and programs and much more.

Amber works primarily with Children with Complex Medical Conditions and people with Intellectual and Developmental Disabilities. She is always open to learning from those she works with and willing to share her own personal experiences. Amber has an Associates of Applied Science degree and Bachelor of Health Science degree in Human Services with an emphasis on Family and Youth Services from the University of Alaska Anchorage.

Care Coordination Supervisor
Audri connects individuals from all walks of life to the services they need to live a full and happy life. She values face-to-face interaction with each individual and their entire team. She knows that the individuals and family members who have been there since day one have voices that need to be heard.

For six years, Audri worked as a Direct Service Provider and volunteered with Special Olympics Alaska Bowling Team. She was a partner and mentor for athletes but admits that most of the time, they were teaching her!

Audri has an immense amount of experience working one-on-one with people who experience intellectual and developmental disabilities. She has a certified nurse assistant license and is working towards a degree in nursing.
