What is Care Coordination?

It can be difficult and overwhelming determining what services are available and how to access them. Care Coordination services relieve some of that burden by guiding you through the maze of available health care, financial and social services.

• We identify and connect you with the services and supports you need to maximize your independence and ensure your safety.

• We provide you with the information you need to select your own services - a process that need not be difficult or overwhelming.

• We advocate with community organizations to support you in reaching your goals.

• We serve anyone with a disability including those with Alzheimer's disease and related dementia (ADRD), Intellectual and Developmental Disabilities (IDD) - Alaskans Living Independently (ALI) - Adults with Physical and Developmental Disabilities (APDD) - Children with Complex Medical Conditions (CMCC) and frail elders.

Not all services are available statewide, contact your local office to find out options are available in your area.

Our Care Coordinators

are professionals who have extensive and current knowledge of community resources. They know eligibility requirements and payment options for long-term care alternatives and can assist you with planning and decision making.

All Care Coordinators at Alzheimer’s Resource of Alaska are certified by the state and have specialized knowledge of community resources .

Care coordinators work as the link between you or your loved one and the numerous services they receive.

Care Coordination services are for those who require more than one service at a time or for those who need help accessing the Alaska Medicaid Waiver Program.
