Money for services and items is available for people with Alzheimer’s disease or related dementia (ADRD). The grants must provide individuals diagnosed with ADRD essential items which will directly improve their quality of life and increase their independent functioning. These can include medical, dental, vision, hearing services, supplies, therapeutic devices, adaptive equipment, and accessibility improvements or supplies and services not available from any other funding source.
A person may apply for a mini-grant as a beneficiary (person affected by the disorder) or as an applicant (a family member, care coordinator, or guardian filling out the form on behalf of the beneficiary). The mini-grant program is funded by a grant from the Alaska Mental Health Trust Authority and grants are distributed by Alzheimer’s Resource of Alaska.

1. Submit a completed mini-grant application. You may need Acrobat Reader to view & print this application.
2. Attach a written estimate from the vendor for the cost of the item(s) or service(s) to request for the beneficiary.
Mini grants are awarded monthly. Applications must be received by the close of business on the first Tuesday of each month. If the deadline falls on a holiday, the deadline rolls to the next business day.
